Time only moves forward, so considering you had X outcome from Y action, you can't go back and have Z outcome from Y action. Not just time but infinite other variables can't be replicated so that you have Y action again and get =/= X result. Therefore it's fair to say that Y action always gives X outcome.

"Did you actually take a risk?" makes risk out to be something "real" or objective, when it's an entirely subjective value that actors attach to actions. It's like saying, "If you thought a song was good, and then think it's bad, was it actually good?"
