Since it's definitely impossible to control my appetite, the only remaining option, apart from surgery and medication, is to purge whatever I've eaten down the toilet. Could this be it?

You cant control your appetite but you can control how to use food in your daily life. Dont purge, its not a means to achieve anything permanently + has infamous nasty side effect
Here are some of my meditations in my notes on controlling calorie intake in your daily routine that exploit your brain (AI bots say its all scientifically correct but idk how trustworthy that is). Sorry if this is all useless and u know it already, just sharing in case it helps.
Reacting to cravings
Cravings can be real or passing thoughts. The idea of a food itself can trigger hunger and lower impulse control. Resisting such thoughts usually lets them pass, revealing a lack of hunger in reality. But since reasoning abilities seem to drop during this process, for now it would seem this is very hard to control. It might be best to think of cravings as a distraction, and one should attempt to get themselves busy with something else ASAP
What to do once you give in
Giving in to cravings doesn't have to be catastrophic. You might need to limit insulin spikes to the fullest at that point, though. I noticed, eating something when you have a craving tends to trigger an additional desire for food afterwards. Both seemed to be passing. But to simply give in, with a low-fiber, calorie-dense food seems to be bad. It would be best to eat something like a fruit at this point. Or maybe a fiber-rich bar.
Restrict ability to indulge
Limit access to food. When one's really hungry, reasoning abilities seem to drop. Access to junk food needs to be restricted. The only way to eat junk food, should be to go and buy it now, in the moment, so there's work involved.
Structuring meals for satiation
You can structure a meal to be controlling for total calories eaten. Starting off with the protein-rich foods, like the eggs in a breakfast, is the best way. They're easier to consume when one's at their hungriest. Then, you would consume a bit more fat together with them. Like the bacon in breakfast. Then it would probably be important to be having a steady intake of fiber throughout (or perhaps after) this part of the meal. And finally, you would also consume carb-heavy food, with fiber, so the insulin spike is lower.
About cooking your food
Cooking your own meals allows for much better calorie tracking. You can measure every single ingredient that way. Also, it seems like even looking at food triggers something within us, as does thinking of it. I noticed its extremely hard to resist food, whenever you're hungry, and it's right in front of you. I don't know what the mechanism behind this is, but the prolonged looking at food ingredients during the cooking process might be a way to mess around with it.
How to eat meals to calorie restriction.
Taking time to eat meals is better. Initial cravings for food can be overwhelming. The fullness of the stomach is not directly aligned with hunger. A big and real craving for a meal, is followed by a bigger insulin spike, i would imagine. Which signals to someone that they should indulge as much as possible now. Therefore, one eats even after their stomach is full, due to this dissonance between the stomach and the brain. So it would be best to eat slowly, so you dont keep eating too much after you no longer need to. Maybe we don't have control over this, maybe reasoning abilities drop in this case as well. Perhaps this is why we have dinner with our family. Resisting a conversation, even while eating a delicious meal, seems to not be that easy. Or maybe its best to eat when you have something to distract you, so the eating speed is slower, and you can feel full whenever you need to.
I find using these tips to have helped me a ton, weight loss is about cognitive science shit mostly, we tend to become unreasonable when food and hunger are involved.
